Community Assistant (CA)
Community Access Individual (CAI) Services
Nature of Work
To provide supervision and support with individual community activities. To work with individuals on the CAI stated goals of their individual service plan.
Creative Community Services (CCS) will maintain a personnel file for compliance. The documentation required:
The CA understands that CRP first aid, CCS orientation are required prior to working with an individual.
CCS will monitor training requirements and must keep required training information on file. The CA will complete the following required trainings within 30 to 60 days of working with any CCS individual
Provide supervision, transportation and supports related to the CAI goal for the individual. This may include but is not limited to life skills, approved goals by DBHDD, and independent living tasks. The CAI service requires transporting the individual out into the community to do social activities related to the CAI goal.
The CA is responsible for documenting a daily log and goal tracking on the say the service is completed. This documentation will be submitted to the CA supervisor, the Family Consultant. The CA will submit a timesheet for services provided on the timeline given by their supervisor.
The CA will work with the individual according to approved weekly hours from the supervisor.
Approved CAI hours vary between 7 to 30 hours per month. The CA will receive confirmation from their supervisor regarding approved hours on a weekly basis.
The CA may also assist with caregiving in the home on a case by case basis.

- Copies of social security cared
- Copes of driver’s license
- Proof of liability insurance
The CA understands that CRP first aid, CCS orientation are required prior to working with an individual.

- IDD Supports
- Medication
- Personal Care Assistance (PCA)
- CPR First Aid
- Safety Care
